Persons with mental illness die an average of 25 years younger than the rest of the population. To help improve this population’s longevity, the Mental Health Transformation (MHT) Project, a partnership between the Governor’s Office, the Texas Legislature, state agencies, federal entities, and consumers, has allocated resources to advance the integration of physical and mental health care at the local level with an initial focus on five Community Collaboratives across Texas.
Within each Collaborative, the Local Mental Health Authority (LMHA) is working in partnership with Federally-Qualified Health Centers (FQHCs) and other organizations to develop and implement plans that accelerate integrated care between health care providers.
To assist with this integration, Collaboratives participating in the initiative are being provided a web-based Health Assessment that helps identify an individual’s risk for conditions including diabetes, cardiovascular disease, smoking, alcoholism, substance abuse, obesity, and depression.
In addition to the Health Assessment, the MHT Project is also providing resources to assist clinics and care sites in modifying workflow processes and coordinating care with other organizations within the community.

Integrated Care Workbook Material
Dartmouth-Hitchcock has developed a resource called clinicalmicrosystem.org to help improve the delivery of medical care. Included in the resource material are workbooks and worksheets that Collaboratives can use to enhance and integrate service delivery systems in a patient-centric fashion. A consultant from the MHT team, Laura McKieran, will be working with each Collaborative to address each Collaboratives’ specific needs. Laura can be contacted at laura@helpinggoodgrow.com.
